Menstrual cups, pads and tampons all manage menstrual flow, but they differ in where they are worn, how they are changed and what facilities they require. The best option depends on the person, the day and the setting. This evergreen comparison replaces claims that one generation is leading a “period revolution” or that one product is always healthier, cheaper or greener.

Quick comparison

Factor Cup Pad Tampon
Use Internal; collects flow External; absorbs flow Internal; absorbs flow
After use Empty, clean and reuse Usually discard Discard
Learning Folding, insertion and removal Placement and absorbency choice Insertion and absorbency choice
Facilities Handwashing, cleaning and storage Disposal bin Disposal bin and handwashing

How each option works

A cup sits in the vagina and collects flow. It must fit comfortably, open correctly and be removed for emptying and cleaning. Pads attach to underwear and do not require insertion. Tampons absorb internally and are available in different absorbencies. Many people combine products—for example, a cup with a liner while learning or pads on days when an internal product is not preferred.

The 2019 systematic review of menstrual cups found that familiarisation often took several cycles. Leakage in the limited direct comparisons was similar to or lower than with pads or tampons, but the evidence base had important limitations. That does not support a leak-free guarantee or a claim that cups suit everyone.

Cost depends on real use

A reusable cup has a higher upfront price than one pad or tampon. Whether it saves money depends on how often it is used, how long it remains suitable, local prices, cleaning costs and whether backup products are also purchased. With proper care, Elys Cup can last up to 5 years, but it should be replaced earlier if it deteriorates. Do not turn that maximum lifespan into guaranteed savings.

Environmental impact depends on assumptions

A cup can reduce the number of disposable products purchased and discarded. A life-cycle assessment dataset compared representative pads, tampons and cups across materials, production, use and disposal. The associated research reported lower modelled impacts for the reusable cup in the scenarios studied. Results depend on assumptions such as lifespan, cleaning energy and water, frequency of use and the products replaced. They are not proof that every cup user has the same footprint. See the life-cycle inventory and assessment dataset.

The 2019 systematic review also included preliminary cost and environmental estimates and explicitly called for better comparative research. Avoid “zero waste”, universal savings and exact replacement counts unless a transparent product-specific method supports them.

Comfort, access and choice

A cup requires insertion, dexterity and a cleaning routine. Pads may be more practical without private water access or when an external product is preferred. Tampons may be familiar and compact but also require correct absorbency and timely changing. Disability, pelvic health, culture, age, school/work facilities and personal comfort can matter more than marketing claims.

Elys Cup safety basics

Elys Cup is made from medical-grade silicone and designed for removal without a suction effect. It can be worn for up to 8 hours and must be emptied earlier according to flow and the instructions. Wash your hands before handling it, clean it as directed and remove it if it causes pain or persistent pressure.

How to choose

  • Decide whether you prefer an internal or external product.
  • Consider water, privacy, disposal and storage where you spend the day.
  • Compare total routine costs rather than a single unit price.
  • Try a new product on a low-pressure day and keep a familiar backup.
  • Choose without generational, environmental or body-confidence pressure.
